  2. State your name.
  3. What is your address?
  4. Did you receive and read the bankruptcy information sheet? (Yes)
  5. How long have you lived in the State of Nebraska?
  6. Are you employed?
  7. How are you employed?
  8. Are you married?
  9. What is the name of your spouse?
  10. How many other persons are dependent upon you (usually – how many children do you have)?
  11. Do you own a home or any real estate at all?
  12. Have you sold or transferred any real estate or personal property (e.g., cars) in the last 12 months?
  13. Have you recently received an inheritance?
  14. Do you expect to receive an inheritance within the next 6 months?
  15. Is this your first bankruptcy?
  16. If not, when was your last bankruptcy?
  17. Did you read your schedules? Did you sign? Are you personally aware of the information in these schedules? Is the information accurate? Any errors or omissions? Are all of your assets, debts and creditors identified in the schedules?
